  Please complete this form if you intend to transfer out of Purdue University to another U.S. educational institution which is activated in SEVIS (Student & Exchange Visitor System), to notify the Purdue International Student & Scholar (ISS) office of your intent.

F-1 Documents

• Student Request Form
  This form is a multi-purpose form used to apply for the following: travel request; program extension; change of information/replacement I-20 or DS 2019, or; inviting a spouse or child to visit the U.S.

• Curricular Practical Training (CPT) Form
  CPT is an F-1 student benefit. It is authorization for employment which is an integral part of an established curriculum and is related to a student’s major area of study. This can be defined as: alternate work/study, internship, cooperative education (Co-Op) or other type of required internship or practicuum. Click here for detailed information regarding CPT. The following downloadable form is needed to apply for CPT:

• Optional Practical Training (OPT)
  OPT is an F-1 student benefit. It is authorization for employment directly related to a student’s major area of study. Click here for detailed information regarding OPT. The following downloadable forms are needed to apply for OPT:

J-1 Documents

• Student Request Form
  This form is a multi-purpose form used to apply for the following: travel request; program extension; change of information/replacement I-20 or DS 2019, or; inviting a spouse or child to visit the U.S.

• Academic Training (AT)
  AT is a J-1 student benefit. It is authorization for employment directly related to a student´s major of study. Click here for detailed information regarding AT. The following downloadable forms are needed to apply for AT:

• J-1 Student Employment Information Form
  This form is used to authorize hourly (bi-weekly) employment of Purdue University sponsored J-1 students. It is also used to authorize teaching or research assistantships in departments other than the student's home department. This form must be completed and approved prior to commencing employment.
